    There's no international format to inform boss about any on-goings in your office. As you have terminated an employee, just write an e-mail to him that such fellow has been terminated due to his such violation of the company's policies and you have taken the decision to let him go. That's all there is to say on this subject and your boss will surely understand it. You need only to worry if you have terminated his kin that would require you some major explanation.
