Wedding invitations. If she's a doctor & he isn't what is proper. Dr. Jane Smith & Mr John Smith, Mr & Dr Smith, Mr & Mrs Smith?

    Dr. Jane Smith & Mr John Smith

    Mr. John Smith and Dr. Jane M. Smith (if they both use the same surname)

    Mr. John Smith and Dr. Jane Miller (if she uses her maiden name in her practice)

    It doesn't really matter which name is first. Husband then wife is a traditional, acceptable way. The fact that they are shown on the same line indicates that they're a married couple.

    Dr. Jane Smith and Mr. John Smith (assuming they are married and use the same surnames)

    Dr. Jane Smith & Mr. John (whichever name he uses, because she may have kept her maiden name)
