is a person required to have medicare?

    There's several parts. People are automatically enrolled in Part A when they start collecting social security.  I guess you could consider automatic enrollment to be a requirement even though its free for people that have worked at least 10 years (or married with a spouse working at least 10 years).   All other parts (B through D) are optional and they require monthly payments.   A slight majority of people choose to pay for Part B

    Being enrolled in social security causes automatic enrollment into medicare.

    If you are getting social security benefits, then medicare is not an option, it is an automatic part of social security.

    If  you never apply to get social security - then medicare is an option.
