The average person is incredibly lazy with their use of language.  "Gas" instead of "gasoline".  "Weight" when they mean "mass".  "Inertia" when they mean ??? I really don't know this one.  So "has a temperature" means " has a HIGHER temperature than is normal".  One key missing word.

He means he has a fever which is a higher than normal temperature of 98.6 F.

doesn't mean anything.  Closest is "he has a temperature" which means running a fever.
