Because they stick to a very strict diet and exercise - I think the diet is less than 800 calories per day of root veg, Google Newcastle diet and it explains it

If the person gains better control of their blood glucose levels and is able to get their A1c to a low enough level, they may no longer need any meds to help with their BG control.

Each diabetic is different as far as how well they can do with diet and exercise alone. So some may still need meds, while others can go off meds. Some diabetics may not be willing to make the necessary changes in their diet, to not need to use meds. And others, no matter what changes they make, may always need meds.

Diabetics need different levels of medication because everyone is different.

Some people can manage their diabetes by making diet and life style changes and therefore, need less medication. Some people have a bad reaction to certain medications and need to be prescribed something different.Other people do well on metformin but need the highest dosage because diet changes and exercise wasn't enough to help them with their diabetes.  (or maybe they aren't making enough diet changes and just trying to let the medication take care of the issue)Diabetes is not a condition that can be compared.  You should NEVER compare how two different people are dealing with their diabetes because there are too many variables involved in managing diabetes and even too many variables in why each person might have diabetes.  For someone who became diabetic because of obesity, poor diet, and lack of exercise, sometimes dropping weight, changing eating habits, and being a little more active gives them the ability to stop taking medication because they have reversed enough of the reasons that caused them to need medication.  If they change back to their bad habits, it is very likely they would end up back on medication.  If they keep following their new, better habits, they may be able to avoid medication for many years or maybe even the rest of their life.

Metformin has some rather undesirable side effects., and it has fallen out of favor with many doctors .There are other meds available which do not have these side effects
