Just went to the DMV last Thursday.  I used my passport; social security card; bank statement with my name on it (my husband used a homeowner's policy); social security annual statement; driver's license, we're in Illinois.  You can download Real ID Document Checklist from the Secretary of State.

I would check the rules in your state.

No, use a Passport and some kind of utility bill, Electricity, water etc.  A bank statement doesn't prove you live somewhere.

    No.

Current valid ID, Endorsed Birth Certificate, Social Security card and vehicle owner card, valid passport or valid insurance card.

Proof of residency for many things requires at least two items.
