The formula of a parabola in vertex form can be represented as
y = a(x - h)² + k
vertex is (h, k)
From the graph, the vertex is (3, 6)
(y - y_0)^2 = 4 a (x - x_0)
(y - 6)^2 = 4 a (x - 5)
y = 4 (a x - 5 a + 9)

A formula of parabola in vertex form can be represented as
y = a(x-h)² + k
vertex is (h,k)
From graph vertex is (3,6)
y = a(x-3)² + 6
since the parabola passes through point (5,11), it should should satisfy the equation
11 = a(5-3)² + 6
5 = 4a
a = 5/4
so the equation becomes
y = (5/4)(x-3)² + 6
