SIRVA from flu shot?

I had a flu shot almost a month ago and my left arm still has soreness when moving it certain ways, like out to the side, or rotating it, the pain is in the area of the shot. Has anyone else had soreness lasting this long? Should I wait longer or see a doctor soon?

You don't need to see your doctor for that. Call his office, ask to speak to his nurse and explain your situation. Ask her if the continuing soreness is something you need to be concerned about it. She has surely seen this before and can advise you.

I've had it twice in 30 years. The needle hit a nerve, it takes ages to settle down. Try icing it.
