A disc that can't be read by whatever hardware it is in. Such as a cd player, trying to play a dvd or blu-ray.

The player or software cannot interpret whatever data is on the disc.

eg. Trying a disc used to save computer files in a basic video DVD player could give an error message such as that, as the machine can only translate the DVD video_ts format using .ifo & .vob files.

It's a disc that cannot be read by the hardware. 

If it is a burned DVD-R it needed to be finalized most likely.

Could be any number of things, but the disc is not compatible.
