Is it cruel to apply this to a kid?

something that's for his benefit but that same exact thing makes him sick, ie gives him stomach pain

you got permission from his mom to do this annually

There's no needle involved. It's a gel or a paste placed inside the mouth.

And when swallowed, it hurts the stomach.

You clearly witnessed something that's made you uneasy as you wouldn't be posting the same question over and over and over again had you not. So maybe contact another doctor to get a second opinion. Or if you're working for a physician whom you think is acting badly avail yourself of the whistleblower laws and report this to the medical board.
