Is it out of line to ask if there is anything that can be done to help my coworker?
We don't know tons about him but we know that he's in his 20s and was on multiple mental health medications until last year when he lost his health insurance and hasn't gotten it back because he can't afford it so he's gone unmedicated. It seems like that's starting to wear on him because he's lost probably 40 pounds and his mood has changed drastically, like very angry one minute and happy the next and he forgets things now that he didn't before and talks to himself and says weird things and seems like a completely different person than he was this time last year and recently he's been giving a bunch of his stuff to people at work like every day he's got a new thing he's giving away and just says he doesn't want it anymore.
It has a couple people worried I guess but is it something anyone approaches or just stand by and let it go because it's not our business? As far as we know he has no family or friends or significant other to reach out to so we are all he's got

There's nothing "that can be done" about your coworker besides informing your boss regarding his erratic behavior.
